Schedule an Appointment Online

*All  appointment times are Eastern Standard

  • All patients must be either a resident of Colorado or Connecticut. 

  • We only take Cigna, Aetna, Optum, United or Self-Pay Clients​

Booking Instructions:

New Patients

Step 1

Schedule an Intake Appointment

  • Select the Book Now button next to the 60 min Intake service.

  • A calendar will appear, select a day and time for your appointment.

  • Enter your name, email address, and telephone number

Step 2

Complete paperwork 

  • Once you have completed the submission form, you will receive an email with an Intake form and consent paperwork to be electronically signed.

  • Once your consents are signed, you will be asked to register for the patient portal.

  • Within a few days we will verify your insurance coverage and confirm your appointment.

Step 3

Join your intake appointment!

  • Now that your appointment is scheduled and your paperwork is complete, you're ready to meet with Melissa.

  • Leading up to your appointment, you will receive reminders and links to join the telehealth platform. At the time of your appointment click the link and enter the virtual waiting room.

Returning
Patients

Cancel or Reschedule an Exisiting Appointment

  • If you need to make changes or cancel an existing appointment, please select "Manage Appointments" at the top right corner of the booking app.

Schedule a Follow Up

  • If you are an existing patient who has already had an intake appointment and need to schedule a new follow-up appointment, please select the Book Now button next to the 30 min Follow up service.
